Onboarding Document Packet
Once you're hired, complete the documents below before your first day. Government forms link to official sources. Salon-specific forms are Sinkor-branded and printable.
W-4 — Federal Tax Withholding
IRS form to set your federal tax withholding amount. Complete and bring to your first day.
I-9 — Employment Eligibility
USCIS form verifying your identity and authorization to work. Bring acceptable documents (passport, DL + SS card, etc.).
Delaware State Tax Withholding
State of Delaware employee withholding form. Required for all Delaware-based W-2 employees.
Direct Deposit Authorization
Authorize Sinkor Beauty to deposit your paycheck directly to your bank account. Attach a voided check or bank letter.
Emergency Contact Form
Provide your emergency contact information and any relevant medical notes for our records.
Employee Handbook Acknowledgment
Review the Sinkor Beauty Employee Handbook and sign the acknowledgment form on your first day.
W-9 — Request for Taxpayer ID
IRS form providing your TIN (SSN or EIN) for 1099 tax reporting. Required before your first payment.
Independent Contractor Agreement
Sinkor-branded contractor agreement outlining terms, payment schedule, and expectations. Sign before starting work.
Confidentiality & Non-Compete Agreement
Protects client information, business practices, and sets reasonable non-compete terms during and after employment.
Commission Agreement
Outlines your commission rate, payment schedule, and performance expectations. Review before your first day.
